Today an unannounced monitoring visit was conducted with the Provider. This visit took place in the morning between 9:00am-10:45. There were 9 children present with the Provider for a total of 26 points within the supervision guidelines. The children were observed having morning exercise, story-time, arts & craft and in transition for lunch. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu and emergency procedures were observed during this inspection.
Standard #: 22VAC40-111-110-A Description: Based on a review of the children records, three of the records observed did not include documentation of the current Virginia Department of Health form. Evidence: Child #4, 8 & #9's record did not include documentation of the physical exam and immunization record on the virginia health form. Plan of Correction: Provider stated that she will notify the parent and ask them to get the form signed at the next appointment.
Standard #: 22VAC40-111-70-A Description: Based on a review of the children records, one record did not include documentation of the information for parent form. Evidence: Child #8's record Plan of Correction: Provider stated that the form was given to the parent however she have not returned it. She will contact the parent.
Standard #: 22VAC40-111-180-A Description: Based on a review of the caregiver and household records, one record did not include documentation of a repeat tb screening two years from the date of the last screeening. Evidence: Caregiver #6's record did not include documentation of a repeat TB screening. Plan of Correction: Provider stated that she had the tb screening done however the doctor was suppose to mail the results to her. She will have the staff member follow-up or have it done again.
Standard #: 22VAC40-111-540-A Description: tables/high chairs without safety strap securely fastened. Evidence: 2 toddlers were observed in feeding tables/high chairs however they did not have a safety strap fastened. Plan of Correction: Not available online. Contact Inspector for more information.
