Today an unannounced two-month visit was conducted with the Provider. This visit took place in the morning between 9:00am-10:45 there were 4 children with the Provider and 2 assistants for a total of 16 points within the supervision guidelines. The children were observed having morning free play, using gross motor skills, tummy time, free play. Positive interactions were observed during this inspection. Standard #: 22VAC40-111-960-E
Description: Based on observation of the infant bottles, bottles did not include child's full name and date. Evidence: Child #4's bottle did not include the name and or date.

Plan of Correction: Provider stated that she will label it immediately.

Standard #: 22VAC40-111-60-B
Description: Based upon a review of files, children's files are incomplete. Evidence: *1 of 4 files reviewed was missing complete health insurance information. (Child 2)

Plan of Correction: Provider stated that she will ask the parent for the information.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and review of the provider records, the provider did not have the findings from the most recent inspection of the facility. Evidence: Upon arrival, the violation notice dated 5/19/16 was not posted at the family day home.

Plan of Correction: Provider stated that she was only given the license and did not know what the violation notice was. Provider was informed that it must be posted at all times with license.
