Comments:
An initial subsidy health and safety (SHSI) inspection was initiated and concluded on Wednesday, January 11, 2023. There were no children and staff present for observation during this inspection. The SHSI inspection covered the subsidy health and safety regulations. A total of 2 staff's records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.058
Description: Based on interview, the center did not ensure to be equipped with at least one carbon monoxide detector.

Evidence: During the inspection on January 11, 2023, interview with the owner confirmed there was no a carbon monoxide detector in the building for the inspection.

Plan of Correction: The provider will purchase carbon monoxide detector to place in the building.

Standard #: 8VAC20-790-550-2-a
Description: Based on review of staff records and interview, the center did not ensure to obtain results of fingerprint -based national background check.

Evidence: In Staff#1's and Staff#2's records, there were no results of fingerprint-based national criminal background check found. Interview with staff confirmed the results of a fingerprint results were not available for review during the inspection.

Plan of Correction: The provider obtain the contact information for OBI and then scheduled fingerprinting appointment.

Standard #: 8VAC20-790-650-A
Description: Based on observation and interview, the center did not ensure facility have at least two toilets and two sinks.

Evidence: During the inspection, there was observation of one toilet and one sink at the program. Interview with staff confirmed observation during the inspection.

Plan of Correction: The provider will consult with expert to explore additional toilet and sink options.

Description: Based on interview, the center did not ensure all first aid kit contain all requirements in the building.

Evidence: Interview with the staff confirmed that there was no first aid kit, emergency supplies available for review during the inspection.

Plan of Correction: The provider will purchase all required first aid kit supplies.

Description: Based on interview, the center did not have one working, battery-operated flashlight.

Evidence: Interview with staff confirmed there was no working, battery-operated flashlight available for review during the inspection.

Plan of Correction: The provider will purchase flashlight for the center.

Description: Based on interview, the center did not have one working, battery-operated radio.

Evidence: Interview with staff confirmed there was no working, battery-operated radio available for review during the inspection.

Standard #: 8VAC20-790-790-A
Description: Based on interview, the center did not ensure to develop a written emergency preparedness plan for emergency evacuation, relocation, lockdown, and shelter-in-place procedures.

Evidence: Interview with staff confirmed a written emergency plan was not available for review during the inspection.

Plan of Correction: The provider will developed a written emergency preparedness plan for the center and will submit to inspector for further review.
