- Technical Assistance:
Consultation provided on the requirements for an emergency back-up person and reviewing the emergency preparedness plan annually.
An unannounced monitoring inspection was conducted today with the provider, who was present with her assistant, 7 preschoolers ( 2 to 4 years of age) for a total of 14 points. The children were napping during the inspection. For lunch, they were served spaghettios, meatballs, cheese, corn, applesauce, bread and milk. The provider does not administer medication. Six children's files were reviewed as well and three household members files. Arrival time:12:30 pm Departure time: 2:05 pm
Standard #: 22VAC40-111-170-A Description: Based on an interview with the provider, it was determined that a household member had not obtained a tuberculosis test/screening. Evidence: Person A moved into the home on 7/1/16 and to date there was no documentation of a tuberculosis test/screening. Plan of Correction: Person A will obtain the documentation from the doctor.
Standard #: 22VAC40-191-40-D-4-a Description: Based on an interview with the provider, it was determined that a household member had not completed any of the three background checks. Evidence: Household member A moved into the home on 7/1/16 does not have a completed sworn disclosure statement on file which is required at the time a person begins to reside in the home. There were no criminal history record check nor a central registry finding on file within 30 days of beginning to reside in the home. Plan of Correction: We will have household member A send off for the criminal and central registry reports this week and we will get it back in 30 days. They will sign the sworn disclosure statement by 9/29/16
Standard #: 22VAC40-191-40-D-5-a Description: Based on an interview with the provider, it was determined that a household member had not obtained a central registry check within 30 days of turning 14 years of age. Evidence: Household member B turned 14 years of age on 7/28/16 and to date does not have a central registry check. Plan of Correction: We will send off for this check this week.
