1. Reviewed menu requirements. 2. Inspector agreed to send a field trip permission form.
Conducted a unannounced monitoring inspection 10:35-11:15am. Observed 2 children = 5 points with the Provider. Children were having free-play on the mid-level of the home, which the Provider stated was the primary area of care. The lower-level of the home is also set-up for daycare. Earlier in the day the Provider + two children walked to the nearby Walmart to go shopping. There was no menu written/posted, though the Provider stated lunch to be served would include: spaghetti with ground turkey, tomato sauce, parmesan cheese, honeydew melon, and water. Milk is not served due to allergy/health reasons and parent request. There was a sufficient amount of supplies and equipment for the children. 2 of the 3 previously cited violations were corrected, while the 3rd violation was cited again. Standard #: 22VAC40-111-920 Description: Based on observations made and interview conducted, it was determined that a menu of food served for the current one-week period was not completed, or posted in the home for parents to view. Plan of Correction: food menu hang up on the wall [sic]
Standard #: 22VAC40-111-980-A Description: Based on records reviewed and interview conducted, it was determined that written parental permission to take children off the premises of the family day home has not obtained. Evidence: The Provider stated that earlier this morning she walked with the children to a nearby store, and when asked, indicated that she had not obtained written parental permission to do so. No field trip permission forms were found in the children's records. Plan of Correction: Inspector send me the field trip permission form. I gave the form for parent they sign and gave to me. I put all children files [sic]
Standard #: 22VAC40-111-70-A Description: Based on records reviewed, it was determined that 5 of 5 children's records did not include required written information, as outlined on the Information for Parents Form. Evidence: A blank Information for Parents Form was signed by parents in 5 children's records. All areas of the Form were not completed by the Provider. Plan of Correction: I fill up all 5 children [sic]
Standard #: 22VAC40-111-280 Description: Based on observations made, it was determined that a potentially poisonous substance was accessible to the children. Evidence: A bottles of mouthwash was found to be sitting on the sink vanity in the bathroom used by the children. The product label stated, "Keep out of reach of children." Plan of Correction: I Trash it same moment [sic]
Standard #: 22VAC40-111-320-B Description: Based on observations made, it was determined that an electrical outlet in the bathroom used by the children did not have a protective safety cover. Plan of Correction: I fix it
Standard #: 22VAC40-111-430-A Description: Based on observations made, it was determined that a child under the age of 2 years old had access to stairs. Evidence: The installed protective barrier to both the upper and lower level stairs was left open while Child #1, a 17 month-old, was playing nearby. The Provider was providing intermittent supervision during this time, and sight supervision was not maintained. Plan of Correction: I all ready fix same day [sic]
