Technical Assistance:
1. Discussed online training courses currently available. 2. Recommended spray bottles be labeled to identify the liquid contents.

Conducted an unannounced monitoring inspection 1:50-3:30pm. Observed 11 children = 26 points with Assistant #1 and Assistant #2. The Provider was on an upper level of the home upon arrival. Staffing has remained consistent for over a year. Children were napping, washing hands, and eating snack during the inspection. Caregivers were supervising the children appropriately and promptly tending to their needs. Snack served: banana, goldfish crackers, and apple juice. The daycare areas of the home were found to be clean, safeguarded appropriately, and well supplied with developmentally appropriate toys and equipment for the children. A Montessori program is implemented. No medications reported to be on-site. Required documents were posted. Record keeping violations found. Questions about this inspection may be directed to:

Description: Based on records reviewed, it was determined that 3 of 10 children's records did not include documentation that the parent had reviewed the emergency contacts annually. Evidence: 1. Child #5 - The most recent parent signature was dated 5/3/15. 2. Child #8 - The most recent parent signature was dated 9/5/15. 2. Child #10 - The most recent parent signature was dated 8/3/15.

Plan of Correction: All 3 records were signed in the same day of inspection.

Standard #: 22VAC40-111-890
Description: Based on observations made, it was determined that food a child brought from home was not labeled with the child's name. Evidence: A bag of cucumbers in the refrigerator belonging to Child #5 was not labeled with her name.
