Today an unannounced monitoring visit was conducted, 6 children were present with a total of 13 points within the supervision guidelines. The children were observed walking to the park, playing outdoors as well as inside with block and toys. children also had lunch which consisted of pork chops, rice and beans, grape and milk. Positive interaction were observed during this inspection. A complete inspection of the physical plant, menu, fire drill, children and provider records were reviewed. Standard #: 22VAC40-111-60-B Description: Based on review of children's record theName and policy number of the child's medical insurance was not included in file. Evidence: Child #1 was missing policy number. Plan of Correction: Provider stated she will speak with mother of child to obtain policy number.
