An unannounced renewal inspection was conducted on October 18th 2016 between 9:30am and 10:30am. There were 10 children in the direct care of a provider and 2 assistants. Children were observed being diapered, napping, creating art, going on a walk, and being fed a bottle. A sampling of staffing and children's records were reviewed. The menu, evacuation drills, the emergency preparedness plan, supervision, and the physical space of the home were also observed and inspected. Description: Based on observation, emergency evacuation procedures were not practiced monthly. Evidence: On the emergency drill form, July 2016 had documentation of a shelter-in-place drill, but not an evacuation drill. The standard requires an evacuation drill to be conducted each month.

Plan of Correction: We will do the fire drills every month and make sure that we also do the shelter-in-place twice a year.

Description: Based on observation, a child-resistant protective cover was not install on all unused electrical outlets. Evidence: In the crib room an electrical outlet was uncapped.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, 1 of 2 records reviewed did not have documentation of a repeat background check before 3 years since the dates of the last checks. Evidence: Staff #1 last had a Central Registry Check on 8/9/2013. In an interview with the provider it was stated that the background checks were submitted prior to the expiration date and had to be sent back to the checking agency 2 separate times.

Plan of Correction: I will contact the office to make sure I get it as soon as possible.
