Comments:
An unannounced monitoring inspection was conducted today. The provider was caring for 5 children, totaling 14 points. The children were observed playing with toys and interacting with the provider. This inspection was completed between approximately 10:00 am and 11:00 am. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-920
Description: Based on a review of administrative documents, it was determined that the posted menu was not completed as required. Evidence: 1. The posted menu was not dated to reflect the current one-week period. 2. The provider did not update the posted menu to indicate today's food substitutions.

Plan of Correction: 1. I will date the menu each week before I post it. 2. I am swapping the food and dinner menus today. I will remember to update the posted menu when I make changes.

Standard #: 22VAC40-111-960-E
Description: Based on observations, it was determined that four bottles of milk were not labeled as required. Evidence: Four children's bottles were not labeled and dated with each child's name.

Plan of Correction: I will label the bottles today.

Standard #: 22VAC40-111-50-A
Description: Based on a review of administrative documents, it was determined that the provider did not complete the written record of children in attendance today. Evidence: Upon arrival, there were 5 children present and in care. However, the provider had not documented any names on the written attendance.

Plan of Correction: I will remember to mark the attendance as each child arrives.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #3 does not include a second emergency contact person.

Plan of Correction: I will ask the parent for the missing information.
