54.1-3408 Provider must be MAT certified to administer prescription medication.

Today an unannounced monitoring inspection was conducted from 1:40pm-3:00pm. There were 3 children(totaling 8 points) directly supervised by 1 staff. The home, 1 staff record, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and at nap. Hand washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Description: Based on record review, 3 of 3 children's parents did not document annually reviewing their child's emergency contact information. Evidence: 1. Child #1's emergency information was last reviewed on 11/21/14. 2. Child # 2's emergency contact information was last reviewed 10/15/15. 3. Child # 3's emergency contact information was last reviewed 3/15/14.

Plan of Correction: I will have parents review information today.

Standard #: 22VAC40-111-530-F
Description: Based on direct observation, an infant had a blanket in the crib and on the outside of the crib, while the child was sleeping. Evidence: 1. Child #2 had a blanket in the crib, while sleeping.

Plan of Correction: The provider moved the blanket,s during the inspection.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, the provider did not have a central registry(CPS) and criminal history record check (CRC) completed within in the past three years. Evidence: 1. The provider's CPS documentation was dated 6/25/13 (expiring 6/25/16) and the CRC was dated 6/12/13 (expiring 6/12/16).

Plan of Correction: I will send these forms out today.
