Technical Assistance:

An announced initial inspection was conducted on Friday, October 21, 2016 from approximately 10:00am until 12:15pm. The provider has requested a license to provide care for 12 children, from one year to 5 years of age. A complete walk through inspection of the home and outdoor areas was conducted with the provider. At the current time, there are no children in care. The background checks for the provider and two household members were reviewed. Pets are present but not accessible to the children. The rabies vaccinations for the provider?s pets were verified.

The majority of care and supervision of children will take place in a den that has been converted to a playroom with a bathroom for the children to use off of the hallway. An additional bedroom will be used for napping if needed. There is a sufficient number of toys and equipment for the requested number of children. Food will be provided and a monthly menu was reviewed during the inspection. The provider stated medication will not be administered at this time. Transportation will not be provided at this time.

Information and proof of correction of violations cited during the initial inspection on October 21, 2016 was sent to the licensing inspector on October 25, 2016. The provider has submitted all of the required application materials at this time. Though, some standards of care could not be determined during the initial study, the provider is in substantial compliance with the standards that could be determined. A recommendation for a six-month conditional license will be made at this time. During the conditional period, the provider will be subject to unannounced licensing inspections, with the first inspection taking place within the first sixty days of licensure. Standard #: 22VAC40-111-770
Description: Based on observation and interview, the provider did not ensure a battery operated weather band radio was present during the inspection. Evidence: During the inspection, the licensing inspector requested to see the battery operated weather band radio. The provider had a radio but the radio was not a weather band radio.

Plan of Correction: Per provider - I did not realize the radio needed to be a weather band radio. I will purchase a weather band radio.

Standard #: 22VAC40-111-800-A
Description: Based on interview and documentation review, the provider did not have a written emergency preparedness and response plan that addressed the features required by the family day home standards.

The licensing inspector was reviewing documentation during the October 21, 2016 initial inspection. The licensing inspector requested to review the emergency preparedness and response plan. The provider stated the template had not been received or completed.

Plan of Correction: Per provider - I do not believe I received that form with the other forms that were sent to me by the department after completing Phase II training. I will complete the plan.

Standard #: 22VAC40-111-940-B
Description: Based on observation, the provider did not have an operable thermometer available to monitor the refrigerator and freezer compartment temperatures. Evidence: During the inspection of the kitchen, the licensing inspector asked to see the thermometer that will be used to monitor the refrigerator and freezer's temperatures. The provider stated there was not a thermometer for those appliances.

Plan of Correction: Per provider - I will buy a thermometer for the refrigerator/freezer and place it in the appliances.
