An unannounced monitoring inspection was initiated today at 12:00pm. The physical plant, 4 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 4 children (plus the Provider's child), totaling 17 points, under the supervision of 2 caregivers. During the inspection, children were observed enjoying free play and resting at nap time. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served chicken noodle soup, cucumbers, tomatoes and milk. The Provider is currently certified in MAT (medication administration training), CPR and First Aid. Should you have any questions, please contact me at email@example.com.
Standard #: 22VAC40-111-90-C Description: Based on file review, 1 out of 4 children's records does not have documentation of additional immunizations every six months up to 2 years. Evidence 1. Child #1is 2 years old. The most recent immunization record was updated when the child was 8 months old. Plan of Correction: The Provider will ask the parent for this.
Standard #: 22VAC40-111-180-A Description: Based on file review, 3 out of 4 caregiver/household members do not have documentation of a TB test/screening completed within the past two years. Evidence 1. Caregiver/household member #2's most recent TB test/screening was completed on 7/8/14. 2. Caregiver/household member #3's most recent TB test/screening was completed on 10/17/14. 3. Caregiver/household member #4's most recent TB test/screening was completed on 7/3/14. Plan of Correction: These will be completed again.
Standard #: 22VAC40-111-280 Description: Based on observations, not all cleaning agent/disinfectants were inaccessible to children in care. Evidence 1. In the bathroom that the children use, there were cleaning wipes left on the lid of the toilet. Plan of Correction: Corrected during the inspection.
