An unannounced monitoring inspection was initiated today at 3:20pm. The physical plant, 5 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 10 children, totaling 31 points, under the supervision of 3 caregivers. During the inspection, children were observed taking naps, waking up from nap time and enjoying free play. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served meatballs on pizza, meatballs with noodles, mixed vegetables and milk. The Provider is not currently certified in MAT (medication administration training) and has a policy that medications will not be administered at the FDH. Standard #: 22VAC40-111-90-C Description: Based on file review, 3 out of 4 children's records do not have immunization records updated every six months up to the age of 2 years. Evidence 1. Child #1 is 2 years old. The most recent immunization record was completed when the child was 7 months old. 2. Child #2 is 2 years old. The most recent immunization record was completed when the child was 15 months old. 3. Child #1 is 17 months old. The most recent immunization record was completed when the child was 9 months old. Plan of Correction: The Provider will ask the parents for these.
Standard #: 22VAC40-111-550 Description: Based on observations, a child in care was using a play pen as a designated sleeping area. Evidence 1. The LI arrived to the FDH at 3:20. The children were sleeping during naptime. One child was observed sleeping in a play pen. Plan of Correction: The child will sleep on a mat or in a crib.
