Technical Assistance:
Your license expires 10/29/16. At this time, your renewal application has not been received in the Eastern Licensing Office. Please ensure that it is received prior to expiration of your current license.

Comments:
An unannounced monitoring inspection was conducted from 1:00 p.m. to 2:30 p.m. At the time of entrance, two 2 1/2 year old children were in care with the provider for a total of 4 points. The records sample size consisted of 2 children?s records and 2 provider / household member records. Children were observed playing with a variety of toys and manipulatives, singing songs and having discussion with the provider. Restroom procedures, hand washing and clean up for transitioning to outdoor play were observed. Medication administration was not observed. The licensee stated that there are no children in care at this time being adminstered medication. Transportation is not routinely provided. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance. The licensee provided verbal responses to the violations during the inspection, which were entered on to the violation notice by the Licensing inspector.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on a review of records and interview, the licensee did not annually review with each parent the emergency contact information required for their child to ensure the information is correct and obtain the parent's signed acknowledgment of the review. Evidence: The records for both children in care (child#1 and child #2) were reviewed. Child #2's record did not contain documentation that the parent had reviewed child #2's emergency information and signed acknowledging said review.

Plan of Correction: The licensee stated that she would have the parent of child #2 review the paperwork, make any required updates and date and sign the review today. Children's records will be reviewed and signed off on by parents annually.

Standard #: 22VAC40-111-800-B
Description: Based on a review of documentation and interview, the licensee did not annually review and update the Emergency Preparedness plan and document said review on the written plan. Evidence: The family day home's written emergency plan had the most recent review documented on it which was dated 1/20/15. The licensee acknowledged that this was the date that the plan had last been reviewed and/or updated.

Plan of Correction: The licensee stated that she would review and document that the plan had been reviewed today. At least annually, the licensee will review and document the review and any required changes at that time.
