Discussed with the provider: Assuring that the attendance sheet is completed as children arrive.
Unannounced monitoring inspection was conducted beginning at 2:30 and ending at 3:45. There were 4 children present with the provider. The children were ages 1 - 3. Children were observed at nap time and playing as they awakened. The youngest was sleeping in a crib and the others were on cots. Reviewed required posting and all were in compliance. Reviewed children's files - see Violation notice. Emergency escape drills are current. Standard #: 22VAC40-111-50-A Description: Based on observation and interview with the provider it was determined that the attendance sheet was not current. Evidence: The provider had an attendance sheet from last week but had not done one for today. Plan of Correction: The provider will write children's name on the form as they arrive and will cross them off when they leave.
Standard #: 22VAC40-111-60-B Description: Based on a review of the files for the four children who were present today, it was determined that 2 files were not complete. Evidence: The files for Child C does not have a physical exam - date of enrollment is 10/28/15. The file for Child B whose date of enrollment is 9/12/16 does not have a physical or shot record, parent's work phone numbers, medical insurance policy number., 1 emergency contact only and that one has only a partial name and no phone number, , no sugned emergency medical authorization. Plan of Correction: The provider will notify parent's of the missing information and give them a date by which everything must be on file.
Standard #: 22VAC40-111-240-A Description: Based on observation, it was determined that not all equipment used by the children was maintained in a safe condition. Evidence: There were two mesh sided play yards that are used for the youngest children to play in at times. These two play yards both had holes in the mesh that could cause an entrapment. Plan of Correction: The provider will get rid of both play yards.
