Technical Assistance:
Discussed the use of highchairs that are meant to be attached to a chair on the floor . Discussed use of space, combining group when one staff is not present.

The provider is present with one assistant and 11 children. A total of 30 points at this monitoring inspection .The children were waiting for lunch when the visit started. After 20 minutes the children had carrots, apples and pizza for lunch and milk was offered as a drink. The children were divided up in three groups. The infants were sleeping and playing on the floor. The younger group were reading and playing with books and the older group was sitting by the table waiting for lunch. rest time was also observed. Lots of singing and conversation between the provider and the children. The provider has lots of supplies for the children to explore, but says she does not currently have the children play in the large playroom with most of the supplies. The children just started this year and will use the supplies in the wrong way. Children choose an activity and bring that to the living room carpet where they do their work. One caregiver is missing today. This visit was started at 12 and ended at 2:30. Please call me at 703-203-8659 if you have any questions regarding this visit.

Description: All items needed in the first aid kit can not be located. Evidence: Based on the review of the first aid kit the provider is missing two triangular bandages and band aids.

Description: The provider failed to ensure that the bottles brought from home are dated. Evidence: Bottles of breast milk has the name of the children but not the date.

Description: The provider failed to get a second emergency contact for one child in care. Evidence: Based on the review of four children's records, one child , child D is missing the second emergency contact.

Plan of Correction: I will ask the parent for that.

Description: The provider failed to get proof of ID for one child out of 4 files checked during this visit. Evidence: Child B is missing proof of ID.

Plan of Correction: I have it on my phone and will add it to the file

Description: The provider failed to get a required paperwork for an employee caring for the children. Evidence: Based on the review of 3 caregiver records, one caregiver, caregiver A, a second reference.

Plan of Correction: I will ask her to give me a second reference.

Description: The provider failed to get a tb screening for one staff in care. Evidence: Based on the review of 3 staff records, one staff, staff A is missing an initial TB screening. Staff started working 7/27/2016.

Plan of Correction: The assistant has done it and will bring in the paperwork.

Description: 2 children do not have rest mats and are sleeping on blankets on the carpet. There is not an inch of cushioning.

Plan of Correction: I ordered new mats already.

Description: The diaper changing mat is ripped and can not be sanitized without the foam being exposed.

Plan of Correction: I will get a new mat.

Description: The provider failed to have a foot operated trash can for diapers. Evidence: Based on the physical plant inspection the trash can in the infant area is hand operated.

Plan of Correction: I have been looking for one and will get one.

Standard #: 63.2-1701-F
Description: The provider failed to post the license and the most recent violation notice in a place conspicuous to the public. Evidence: The license is posted in the providers laundry room and the provider stated that parents do not regularly come by the board. The most recent violation report is not posted on this board.

Plan of Correction: I will redo my parent board and move it to a place where parents can easily see the license and the most recent violations.
