63.2 Liability Insurance Disclosure
- Technical Assistance:
The licensing inspector and licensing administrator conducted an unannounced monitoring inspection on November 18, 2016 from 11: 00 am to 1:20 pm. During this inspection, there were two children observed in care totaling five points with three caregivers. The children were observed in the designated area playing with age appropriate blocks, a toy telephone, and completing puzzles. The provider was also observed practicing the Spanish language with the older child. The inspector conducted interviews with staff and engaged in interactions with children as appropriate. The lunch menu for today consisted of veggie sandwiches, lettuce, tomatoes, chicken meat, and bananas. During this inspection, there was no observation of the children eating their lunch due to it being scheduled at a later time. The children were observed drinking milk prior to lunch. The main room, kitchen area, napping area, bathroom, diapering mat, and play area were inspected during this inspection today. The water temperature was 103.9 degrees. The inspector reviewed four children's records and three records to include the provider and household members. The provider and household members are First Aid/CPR certified and completed annual training requirements. The first aid kit and emergency supplies were also inspected for compliance. The emergency preparedness plan was reviewed for annual update. The last emergency drills and shelter in place drills were completed on 10/21/16. Standard #: 22VAC40-111-60-B Description: Based on review of children's records, the provider did not ensure three of four records contained the parent's signed acknowledgement of receipt of the information required by standard 70.

Standard #: 22VAC40-111-90-C Description: Based on review of children's record, the provider did not obtain documentation of additional immunizations every six months for one child under the age of two years. Evidence: Child #3's record was missing an updated immunization document which is required every six months for children under two. The child is 18 months old and the last documented immunizations were completed on February 19, 2016 when the child was 10 months old. Plan of Correction: Per provider: I will talk to parent and have her bring up to date record by Wednesday, November 23, 2016.
