- Technical Assistance:
An unannounced monitoring visit was conducted today beginning at 11:45am. There were 12 children (40pts) present with 3 caregivers directly supervising. The child to adult ratio was in compliance with standards. The children were observed napping. The physical space, provider records, 3 out of 12 children's records, emergency supplies, evacuation records and menu were inspected. There were 9 cribs available for 7 infants. There was an abundance of developmentally appropriate supplies available and multiple examples of children's artwork were displayed. The facility was clean and organized. Standard #: 22VAC40-111-460-D Description: Based on review of records, the provider did not maintain proper documentation of the current rabies vaccination. Evidence: The available documentation stated the expiration date for the rabies vaccination was 11/04/2016. The updated documentation from the recent visit to the vet was unavailable for review. Plan of Correction: Went to vet to get documentation.
