Discussed the following standard: operational responsibilities.

Comments:
An unannounced monitoring inspection was conducted today. Observed 4 children (10 points) directly supervised by 1 caregiver. Ratio and supervision were in compliance. A sample of 1 child's record, 1 caregiver record, and 2 household member records were reviewed. The physical plant, and emergency procedures were also reviewed. No medications were reported to be on site. The children were engaged in various activities, to include: free play, hand washing, and a walking field trip. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Inspection conducted 11:45am-12:20pm. Description: Based upon documentation reviewed, the provider did not review the emergency plan at least annually and update the plan as needed. Evidence: The emergency plan was last reviewed/updated on 03/11/2015.

Description: Based upon interview, the provider did not maintain an up-to-date record at the family day home for each enrolled child. Evidence: Upon request, the provider was unable to locate the file for Child A during today's inspection.

Description: Based upon documentation reviewed, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (TB) Screening form within the allowable two years time frame. Evidence: 1) Caregiver #2 did not have a current TB screening form on file; last TB screening was dated 03/05/2014. 2) Household member #1 did not have a current TB screening form on file; last TB screening was dated 08/14/2014.
