An unannounced inspection for license renewal was conducted today. Six children were present with the provider and two assistants. Total 19 points with three caregivers. Two children are new since the last inspection. Their records were reviewed today. The children were eating lunch, learning some Spanish words and getting ready for rest time. As we discussed, please review the new assistant's record for all the required information. All the forms you need are available on our website. Standard #: 22VAC40-111-70-B Description: Based on record review, there was no parent's written acknowledgement that they received all the required written information from the provider for child #1 and child #2. Plan of Correction: All written information will be provided to the parents and documentation that they received it will be obtained.
Standard #: 22VAC40-111-80-C Description: Based on record review, there was no documentation that verification of age and identity of child #1, was obtained. Plan of Correction: Verification will be obtained and documented.
Standard #: 22VAC40-111-90-A Description: No documentation of immunizations was found for child #1, who started in care on 9/26/2016. Plan of Correction: Documentation of immunizations will be obtained and kept on file.
Standard #: 22VAC40-111-200-C Description: Based on record review, there was no documentation of orientation training for the assistant who started in March, 2016. Plan of Correction: The orientation training for this assistant will be documented and kept on file.
