Discussed with provider: White Delta crib may not be used until a certificate of compliance can be obtained. If no certificate is obtained remove the crib - do not use until that time.
An announced Initial inspection was conducted today. There are 4 children present with 2 adults. 6 adult and 4 children's files were reviewed. See violation notice form missing information. All original background documents were reviewed. Children were observed resting, using the bathroom, washing hands, eating snack and playing. There are 3 cribs assembled for use with acceptable manufacture dates. Due to case load adjustments, your next inspection may be with a different inspector. Standard #: 22VAC40-111-120-B Description: Based upon a review of files, staff file are incomplete. Evidence: Staff A's file does not have references on file prior to employment. Plan of Correction: I will do the references for her file.
Standard #: 22VAC40-111-70-A Description: Based upon a review of files, parents have not been provided all required policies and procedures in writing. Evidence: Missing information includes but may not be limited to: termination policy, check in and out procedures, schedule of the day. (Children 1,2,3,4) Plan of Correction: I will complete the model DSS form already in use, and have the parents initial changes.
Standard #: 22VAC40-111-320-A Description: Based upon direct observation, small appliances were plugged in while not in use. Evidence: A toaster and coffee maker were plugged in , in the space used by the tenant. Plan of Correction: I will remind the tenant to unplug the appliances. Unplugged during the inspection.
