- Technical Assistance:
A scheduled modification and monitoring inspection was conducted Thursday, October 6, 2016 from 9:45am to 10:45am by the licensing inspector. The provider is requesting to modify the license due to a change of location. The request to modify the location of the family day home was received 09/15/16. The children were finishing breakfast and watching a movie upon the inspector?s arrival. Today, there were two children, totaling six points, present with one caregiver. The provider?s home is a split level with the lower level being utilized for child care. The home has two bedrooms, a living room, bathroom, kitchen and dining room on the main level. The lower level has three bedrooms a family room and a bathroom. The home also has a fenced in backyard Two children?s records, one caregivers? record and one household record were reviewed during the inspection. Medication is not being administered and was reviewed. Transportation is not provided. The provider does not have pets The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-730-D Description: Based on observation on 10/06/2016, the provider did not ensure that medications for household members shall be kept in a locked place. EVIDENCE: A bottle of prescription medication was observed in a side pocket of a bag in the kitchen. Plan of Correction: Corrected on site, medication locked up.
Standard #: 22VAC40-111-90-C Description: Based on a review of children's records on 10/06/2016, the provider additional immunizations were obtained did not ensure that once every six months for children under the age of two years. EVIDENCE: The only documentation of immunizations in the record of Child #2, who is 13 months old, is dated 12/09/2015. Plan of Correction: Will text parent today to bring tomorrow.
