Technical Assistance:
Information provided to Provider regarding the zoning process in order to increase licensed capacity. Clarification regarding opportunity for outdoor play activity depending on weather.

Comments:
The following inspection took place between the hours of 9:15am through 11:10am . Today, I met with the Provider on site at her Family Day Home (FDH) located in Alexandria, Virginia. At the start of this unannounced renewal inspection there were a total of 3 children (11 points) with 1 caregivers. A total of 3 children?s files and 1 staff and 2 household member?s files were reviewed. The children were engaged in the following: free play, stacking blocks and raceway track. Lunch planned today consists of fruit, chicken and mashed potatoes. Organized diapering station was maintained. Standard #: 22VAC40-111-770
Description: Based on walk thru of physical plant, the battery operated weather band radio was not operable. Evidence: Provider had a battery operated weather band radio but the batteries were inoperable. Provider also had a crank radio which is not acceptable.

Plan of Correction: Provider will replace the batteries in the weather band radio.

Standard #: 22VAC40-111-100-A
Description: Based on review of children records, history and physical not obtained within 30 days of first day of attendance. Evidence: In reviewing all 3 children records, Child #3 history and physical was dated 5/13/2016, first day of attendance was 03/07/2016.

Plan of Correction: Provider will ensure that all history and physicals are obtained within the required time frame.

Standard #: 22VAC40-80-120-E-2
Description: Based on walk thur of physical plant, the most recent licensing inspection was not posted. Evidence: Provider did not have the last licensing inspection from June 2016 posted.

Plan of Correction: Provider will post the licensing inspection from today and ongoing.
