An unannounced monitoring inspection was conducted today from approximately 10:45am-12:00pm. There were 2 children(18 points) directly supervised by provider and assistant. The home, 1 staff records, 3 children?s records, 2 household member?s evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Description: Based on review, children records did not have all required information Evidence: Child 1- Missing: parent work phone number, proof of identification, 1 of 2 emergency contacts. Child 2- Missing: parent work phone number, proof of identification, 2 of 2 emergency contacts, parent work number and address, parent address medical insurance policy number physical and immunization. Child 3- Missing: 1 of 2 emergency contacts.

Description: Based on inspection of Family Day Home (FDH) cleaning agents and deodorizers were accessible to children. Evidence: During inspection the laundry room in the area where care was provided was unlocked. A deodorizer was plugged in an outlet where care was provided

Plan of Correction: During inspection provider locked laundry room door and removed deodorizer.
