An unannounced monitoring inspection was conducted today, December 13, 2016 to determine the providers' compliance with the Licensing standards. The inspector arrived at approximately 3:25 pm and concluded the inspection at 5:00 pm. Upon the inspectors arrival, one family day home child and two of the provider's own children were in care with the one provider. A point value of one was calculated today. Children had arrived from the public school and were playing with clay. The provider was participating in the activity with the children. Children made "minions" characters with the clay. Later children washed their hands and had a snack that consisted of a banana, cheese and crackers and a yogurt. The provider interacted with children in a positive manner. All areas of the family day home were inspected. The home has age and stage appropriate toys and materials in operable condition for children in care. The first aid kit along with non-medical emergency supplies, and the emergency preparedness plan (see violation notice) were all reviewed and checked for compliance. The family day home had the daily attendance, schedule, menu, License, and emergency numbers posted. A smoke detectors and fire extinguisher were observed. The inspector interacted with both the provider and children in care. One caregiver record and one child's record were reviewed during today's inspection. Medication is not being administered. The provider has current CPR and first aid training. The last documented emergency evacuation drill was held on 12/01/2016. The two required shelter-in-place drills were completed on 10/17/2016 and 11/01/2016. Transportation is provided. Per the provider: no pets The violations from the previous inspection on March 8, 2016 were checked for corrections. There were no repeat violations found during today's inspection. Description: Based on review of the family Day Home emergency preparedness plan, the provider did not review the emergency plan annually and document in writing each review and update to the emergency plan. Evidence: The most current review of the family day home's emergency preparedness plan was documented on 08/28/2015, exceeding the annual requirement of review.

Plan of Correction: Per the provider: I will review and update my emergency plan tonight.

Standard #: 22VAC40-111-60-B
Description: Based on review of children's records, the provider did not obtain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. Evidence: The record of child #1, did not have written authorization from the parent to obtain emergency medical care should an emergency occur.

Plan of Correction: Per the provider: I will talk to the parent tonight and get her to fill out the emergency authorization.
