An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 6 children, totaling 18 points. This is one more caregiver than is required by the Minimum Standards. The children were observed playing with toys, coloring pictures, and making holiday art. Pleasant interactions between each caregiver and all children in care were observed throughout the course of this inspection. The children were to be served scrambled eggs with ham and tomatoes, applesauce, and milk for lunch later today. This inspection was completed between approximately 9:30 am and 11:00 am. Standard #: 22VAC40-111-750-A
Description: Based on observations, it was determined that two children's diaper ointment products were not labeled as required. Evidence: Two containers of children's diaper ointment were not labeled with each child's name.

Plan of Correction: The ointments will be labeled today.

Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that one record is not complete. Evidence: The file for assistant caregiver #2 does not include: verification of the employee's age; the emergency contact phone number; documentation of completed First Day Orientation training.

Plan of Correction: I will ask her for a copy of her ID; I will ask for the phone number; we did the orientation training but I forgot to mark the documentation. The file will be updated.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #1 does not include: the father's work address; the parent's medical insurance policy number. (repeat violation) 2. Child record #2 and #3 do not include documentation to show that the provider viewed each child's original proof of identification.

Plan of Correction: The records will be updated.

Standard #: 22VAC40-111-530-F
Description: Based on observations, it was determined that soft bedding was placed in the crib with two infants. Evidence: 1. One 7 month old infant was observed sleeping in a crib while wrapped in a thick blanket and with a second blanket draped over the side of the crib. 2. One 10 month old infant was observed sleeping in a crib with a blanket draped over the side of the crib and with a bib secured around her neck.

Plan of Correction: All bedding will be removed from infant cribs. All staff will be reminded to remove each child's bib before placing the child in the crib.
