Comments:
An unannounced monitoring inspection was conducted today from 9:30 - 10:30 to check the new child care area. Ms. Medina has moved the child care area upstairs and now uses the upstairs living room, dining room, one bedroom and the bathroom for child care. The children watched television and played with toys. The exit interview was conducted with the licensee. The violation notice will be e mailed and also mailed by post for completion.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, the licensee did not obtain all of the required information for one child. Evidence - The record for child #5 did not contain the child's doctor's street address., the house number for emergency contact #1 or the child's health insurance policy number.

Plan of Correction: Communicate to parents that they complete the forms

Standard #: 22VAC40-111-240-A
Description: The child care area was not maintained in a safe condition. Evidence - When the licensing inspector arrived, the younger children were seated in high chairs watching the television. There were three cups of hot coffee observed on a child size table in the children's play area.

Plan of Correction: I will not put the coffee back on the children [sic] table.

Standard #: 22VAC40-111-280
Description: Based on observation, the licensee did not store potentially poisonous materials in areas inaccessible to children. Evidence - The storage areas above and below the upstairs bathroom sink, now used by the children, contains RID and hair spray and other toxic products.

Plan of Correction: I'll be safe all the time

Standard #: 22VAC40-111-530-B
Description: Based on observation, the licensee did not have an appropriate sheet on a mattress in one crib. Evidence - There was one crib in use that had a sheet on the mattress that was loose and bunching up and not tight fitting.

Plan of Correction: I'll buy different sheets.
