Licensing Inspector arrived at 10:45 and departed at 11:45 AM. At the time of the inspection, there were 13 children (ages infant through four-year-olds), a total of 32 points with two caregivers. Child were observed participating in free play. The sample size was eight children's records. Violations were found in the area of administration.

Standard #: 22VAC40-111-40-A
Description: Based on observation and interviews, it was determined that the provider did not ensure that the total number of children receiving care at any one time does not exceed the maximum licenses capacity of the home. Evidence: 1. The licensed capacity of the home is 12 children. 2. When the inspector arrived at 10:45 AM, she asked the provider how many children were in care. The provider responded "12". 3. Upon counting the children in care, it was determined that there were 13 children in care.

Plan of Correction: Provider will review schedule for children to ensure that there are never more than 12 children in care at any one time.

Standard #: 22VAC40-111-60-B
Description: Based on a review of eight children's records, it was determined that the provider did not ensure that all the required information was available in each child's record. Evidence: 1. The record for child #6, who was in care at the time of the inspection, did not contain the mother's place of employment and the telephone number for the father's employment. 2. The record for child #7, who was in care at the time of the inspection, did not contain the father's place of employment, the telephone number of the mother's pace of employment, and a second place of employment.

Plan of Correction: Provider will review all children's records to ensure that they are complete.
