Comments:
An unannounced monitoring inspection was conducted today, January 5, 2017 between 10:15am and 11:25am. At the start of this unannounced monitoring inspection there were a total of 11 children (32 points) with 3 caregivers. A total of 2 children?s files and 3 staff/household member?s files were reviewed. The inspector observed children: engaged in indoor gross motor play, art activities, hand washing and eating lunch consisting of chicken, rice and mixed vegetables. and other organized activities. Standard #: 22VAC40-111-240-A
Description: Based on observation, not all areas of the family day home were maintained in a safe condition. Evidence: 1. A lit votive candle was observed in the front foyer area. 2. The candle was in an unattended area of the home that was inaccessible to children in care.

Plan of Correction: The candle was extinguished during the inspection.

Standard #: 22VAC40-111-690-E
Description: Based on observation, the family day home did not dispose of diapers in an approved receptacle. Evidence: There is no diaper recepticle availible by the downstairs changing area that enables diapers to be disposed in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

Plan of Correction: Provider will obtain an use an approved trash receptacle.
