Technical Assistance:
Discussed consolidating and organizing records to prevent oversight of expiration dates.

Comments:
An unannounced monitoring inspection was conducted today beginning at 8:40am. There were 9 children (24pts) with 2 caregivers directly supervising. Ratios were in compliance. The physical space, evacuation drills, first aid kit, 4 children's records, provider records, assistant records and household member records were reviewed. No medications were on site. Children were observed eating morning snack (cheerios, bananas and milk), engaging in free play, interacting with the caregivers and napping (the infants). 4 cribs were present dated September 2011. The site was clean, well organized and contained an abundance of developmentally appropriate supplies and activities. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, it was determined that 3 out of 4 children's records did not include documentation that the parent had reviewed the emergency contacts annually. Evidence: The most recent parent signature of annual review for Child 1 was dated 2014, Child 2 was 2015 and Child 3 was 2015.

Description: Based on review of posted documentation, the provider did not meet the requirements specified in 22 VAC 40-80, General Procedures and Information for Licensure. Evidence: The most recent issued license was not posted on the premises.
