Inspector discussed: ~Preparation of emergency supplies ~Process for moving locations ~Maintaining up to date children's records
An announced initial inspection was conducted today from approximately 11:10am-12:45pm. There were 2 children(4 points) directly supervised by provider The home, 2 staff records, 3 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating lunch. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on review of children records, all required information was not maintained. Evidence: Child 1 was missing 1 of 2 emergency contacts and the address for the the for 1 emergency contact Child 2 was missing health care policy number and addresses for 2 of 2 emergency contacts Child 3 was missing physical signed by a physician. and documentation of annual review. Plan of Correction: Child 1: Emergency contacts and addresses have been completed for both contacts Child 2: Health care policy has been added both contacts are complete Child 3: Physical signed by Dr. will be provided in 10 day 1/29/2017
Standard #: 22VAC40-111-430-D Description: Based on inspection of Family Day Home protective barriers or guardrails were not constructed on the side of stairs. Evidence: The stairs leading to the basement where day care is to be provided did not have guardrail installed Plan of Correction: Guardrail installed on the long wall
