- Technical Assistance:
The assistant should complete a new Sworn Disclosure Statement that is for the correct type of day care program. The provider should have the parents complete a "general field trip" form for walking trips to the neighborhood park. The forms are located on the VA DSS web site under current Family Day Home providers.
An unannounced renewal study was conducted today. Five children were present with two caregivers for fifteen points which is within ratios for supervision. Four children were awake upon the Inspectors arrival and one was sleeping. Children were observed in free play, reading with the caregivers and interactions. Emergency preparation for drills, supplies and plans are current. Lunch to be served today was lentils and rice, vegetables and fruit. The backyard was not reviewed due to the weather. The provider and assistant both have current first aid and CPR certification. The provider has 20.0 hours of annual training for the past year. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-120-B Description: Based on record review, the assistant record was incomplete. Evidence: The second reference was not documented completely prior to employment and was missing the results of the interview. Plan of Correction: A second reference was obtained and confirmed.
Standard #: 22VAC40-111-60-A Description: Based on observation and statement from provider, a record for child #5 could not be located during the renewal study. Evidence: The provider was unable to locate the file for child #5 during the inspection. She looked in all current folders, older ones and in her home. She will continue to look for the folder and contact the Inspector when it is located. Plan of Correction: Provider found the file that evening (1/23/2017) and the Licensing Inspector went by the home to review it on the following day.
Standard #: 22VAC40-111-320-B Description: Based on observation, two outlets in the bathroom were missing protective covers. Evidence: The two outlets did not have child safety covers. Plan of Correction: Outlet covers were replaced.
