Discussion was held on the topics of current enrollment and current staffing.
The following inspection took place between the hours of 11 am through 1 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located near the town of Aldie, Virginia. At the start of this unannounced monitoring inspection there were a total of 12 children (29 points) with 2 caregivers. A total of 4 children?s files and 4 staff/household member?s files were reviewed. The children were engaged in the following: freeplay, bathroom break, diaper changing, hand washing, lunch, games play, toy play, and other organized activities. Standard #: 22VAC40-111-700-A Description: Based on review, prescription medications was given to a child without written authorization from the parent. Evidence - 1. On the date of inspection (01/26/2016), a prescription of a medication to be administered to Child #6 was observed at the Family Day Home (FDH) and written by the child's physician. 2. However, a written authorization from the Parent of Child #6 was not available for review. 3. In addition, an end date for duration or that the medication was to be administered on an "as needed" basis was not specified in the doctor's instructions. Plan of Correction: A written authorization form shall be completed by the Parent of Child #6 and attached to the physician's prescription and instructions.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review, 2 of 4 staff/household member's files reviewed indicated that it has been more than three years since the date of the most recent criminal history record check report (CRC). Evidence - 1. On the date of inspection (01/26/2017), the most recent CRC on file for Member #1 was dated - 11/04/2013. 2. On the date of inspection, the most recent CRC on file for Member #3 was also dated - 11/04/2013. Plan of Correction: New requests will be sent in order to obtain a current report for both members.
