Discussed websites for online training to meet the requirement for annual training.
The following inspection took place between the hours of 9:15am and 11:50am. Today, I met with the Provider on site at her Family Day Home located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 9 children (25 points) with 2 caregivers. A total of 6 children?s files and 2 staff were reviewed. The children were engaged in the following: watching a movie, nice transition to table activities and were very excited about the Valentine party scheduled for the afternoon. Standard #: 22VAC40-111-80-B Description: Based on review of children's records, verification of identity was not found. Evidence: No documentation found that provider verified identity and age for Child #1 and Child #3. Plan of Correction: Provider will request this information from the children's parents.
Standard #: 22VAC40-111-160 Description: Based on review of household members, background checks were not obtained as required. Evidence: Household member #1 and #2 are both 14 years or older. No central registry search was found. Plan of Correction: Provider will send search requests for both household members.
Standard #: 22VAC40-111-200-C Description: Based on review of staff records, documentation of orientation was not found. Evidence: Staff #2 was hired in December 2016. No documentation of orientation training was found. This training is required by the end of the first week. Plan of Correction: Provider will document staff's orientation and maintained in record.
Standard #: 22VAC40-111-210-A Description: Based on review of staff records, 16 hours of annual training was not obtained. Evidence: Provider has only 8 hours of documented annual training. Plan of Correction: Provider will obtain more the required 16 hours of required training.
