**Discussed with the provider that children should not walk around while eating or drinking and should not be allowed to walk around with toys in their mouths.
An unannounced monitoring/renewal inspection was conducted today from 10:10am-11:40am. There were 5 children (20 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, napping and interacting with the caretakers. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-700-A Description: Based on review, the provider failed to ensure that medications are given to children only with the written authorization of the parents and or a physician. Evidence: Reviewed the medication log and found that CH5 is given over-the-counter medication (gas drops) without written authorizations from the parent and or the physician. Plan of Correction: The parent is going to bring the form this evening.
Standard #: 22VAC40-111-900-B Description: Based on observation, the provider failed to ensure that children are not allowed to eat or drink while walking, running, playing, lying down or riding in vehicles. Evidence: Observed that CH1 was walking and running around the playroom while drinking milk from a bottle. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-111-110-B Description: Based on review, the facility failed to ensure that each child's physical examination and immunization record are signed by a licensed physician, the physician's designee or an official of a local health department. Evidence: Reviewed 5 children's records and found that CH3 did not have documentation of a signed health evaluation. Plan of Correction: I will ask "their" mother.
Standard #: 22VAC40-111-60-B Description: Based on review, the facility failed to ensure that children's records are maintained with the required information. Evidence: Reviewed 5 children's records and found that: 1) CH1 and CH3 did not have documentation of medical insurance or insurance policy numbers in their records. 2) CH4 did not have documentation of a second emergency contact. Plan of Correction: I will do it right away.
