An unannounced monitoring inspection was conducted today. Eleven children were present with the provider and two assistants. Total 29 points with three caregivers. The children were eating lunch and transitioning to rest time. The infants were being fed, changed, and playing on the floor. A good supply and variety of age appropriate materials and equipment are available for the children's use. Three children have been enrolled since the last inspection. Their records were reviewed today. The provider is MAT certified, but is not administering any medication at this time. The violations cited at the last inspection have been corrected. Description: Based on record review and provider interview, the record for one enrolled could not be located at the time of the inspection.

Plan of Correction: The record for this child will be located.

Description: Based on record review, there was no documentation that the parents of one child received all the required written information about the family day home.

Plan of Correction: Documentation that the parents received all the required written information will be obtained.

Standard #: 22VAC40-111-80-C
Description: Based on record review, there was no documentation that the provider obtained verification of the age and identity of one child who was enrolled in September 2016.

Plan of Correction: Verification of the identity and age of this child will be obtained and documented.

Standard #: 22VAC40-111-280
Description: Based on observation, the cleaning agents that were stored on the floor of an unlocked closet in the children's play area were accessible to the children.

Plan of Correction: The closet will be kept locked.
