Comments:
An unannounced renewal inspection was completed on 2/23/2017. The inspector arrived at 9:15 A.M. and departed at 11:45 A.M. Seven children were in care at 23 points with the licensee and one assistant caregiver. Two caregiver records were reviewed. Seven children's records were reviewed and two household members records. A physical plant walk-through was done. Discussion: subsidy requirements, standards, and 10 hours in required trainings. MAT training will expire in May of 2017.

Violations:
Standard #: 22VAC40-111-710
Description: Based on review of medications, the licensee failed to ensure that all medications had authorizations. Evidence: 1. The inspector found five prescription medications four did not have authorizations. One medication's authorization had expired 3/13/2016.

Plan of Correction: The licensee printed off new authorization forms and will have the parent to complete the form as well as the physician.

Standard #: 22VAC40-111-720-A
Description: Based on review of medications, the licensee failed to ensure that all medications had authorizations and labeled with the owner's name. Evidence: 1. The inspector found three non prescription medications with no authorizations. 2. 1# medication expired 1/16 , and 2# medication expired 2/16. 3. Medication # 2 and #3 did not have the owners name labeled on it.

Plan of Correction: The medication was discarded during the inspection. In the future the medication will be monitored monthly.

Standard #: 22VAC40-111-730-D
Description: Based on observation the licensee failed to ensure that all medication was locked. Evidence: 1. The inspector observed two medications sitting on top of the lock box in the back room. The children are not allowed in the back room.

Plan of Correction: The licensee locked the medication in the box during the inspection.

Standard #: 22VAC40-111-100-A
Description: Based on review of seven's children's records, the licensee failed to ensure that all physical records were obtained 30 days after starting the program. Evidence: 1. The inspector found that child # 1 started on 8/17/2016 and the physical was obtained on 11/8/2016.

Plan of Correction: The licensee stated that she suspended the care due to the parent's inability to obtain records. The dates were not documented in the record. In the future the licensee will document all dates.

Standard #: 22VAC40-111-110-B
Description: Based on review of seven children's records, the licensee failed to ensure that all immunizations were signed by a licensed physician. Evidence: 1. The inspector found that children # 2 and 3 did not have physician's signatures on their immunization records.

Plan of Correction: The licensee will inform the parent that signatures are needed on the forms and will ensure they are obtained.

Standard #: 22VAC40-111-60-B
Description: Based on review of seven children's records, the licensee failed to ensure that all records were complete. Evidence: 1. The inspector found that children 1, 2 and 3 were missing the previous child care experience.

Plan of Correction: The licensee completed the missing area during the inspection and will monitor all forms.

Standard #: 22VAC40-111-90-A
Description: Based on review of seven's children's records, the licensee failed to ensure that all immunization records were obtained prior to starting the program. Evidence: 1. The inspector found that child # 1 started on 8/17/2016 and the immunizations were obtained on 1/30/2017.

Plan of Correction: The licensee stated that she suspended the care due to the parent's inability to obtain records. The dates were not documented in the record. In the future the licensee will document all dates.

Standard #: 22VAC40-111-680-A
Description: Based on observation the licensee failed to ensure that all children washed hands prior to eating. Evidence: 1. The inspector observed two children arrive and immediately placed at the table and high chair to eat snack without washing their hands before eating snack.

Plan of Correction: The licensee will ensure that all hands are washed before eating.

Standard #: 22VAC40-191-40-D-1-a
Description: Based on interviews and review of household member records, the licensee failed to ensure that all household members had background checks. Evidence: 1. The inspector observed the licensee's sister present living in the home. Confirmed during interviews she has been living in the home for the past two months and will continue for at least the next month. There was no documentation of background checks.

Plan of Correction: The licensee will start the process for the background checks. In the future all household members will have background checks.
