A code compliance inspection was initiated on 3-23-2023 and concluded on 3-23-2023. There were 23 children present, ranging in ages from 4 years to 12 years, with 2 staff supervising. The inspector reviewed compliance in background checks and requirements for child day centers operated by religious institution exempt from licensure. A total of 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.031-A-3 Description: Based on observation and staff interview the vendor did not ensure that when a group of children receiving care includes children from different age brackets, the age of the youngest child in the group is used to determine the staff-to-children ratio that applies to that group.
Evidence:
1. As confirmed by the staff person in charge, there were two staff present with 23 children ranging in age from four years old to 12 years old.
2. Because the youngest child in the group was four years old, the ratio for that group requires one staff member for every 10 children.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22.1-289.031-A-4 Description: Based on record review and interview with staff, the center did not ensure each person in a supervisory position must be certified by a practicing physician or physician assistant to be free from any disability that would prevent them from caring for children under their supervision.
