As discussed the following web sites have free provider online training to be used as a resource:

An unannounced monitoring inspection was conducted with provider at the FDH in Aldie, Va. There were a total of 9 children in care with Provider, 1 assistant and 1 volunteer for a total of 22 points. Walk thru of physical plant conducted. The home was found neat with ample toys and space that were determined age appropriate for the children in care. Observation of free play, soccer, a floor game with a small group of children, diapering, handwashing procedures and transition to naptime. Positive interactions were observed between staff and children. Children had just completed lunch which consisted of macaroni and cheese and hotdogs. A total of 5 children's records and 2 staff records were reviewed. No medications at this time.

Standard #: 22VAC40-111-60-B
Description: Based on record review, some children's records were determined incomplete. Evidence: Child #4's record did not have a second emergency contact. Child #1, #2 and #3's record did not have documentation of an annual review of emergency contact information.

Plan of Correction: Provider will obtain the annual review and the second emergency contact this evening from parents.

Standard #: 22VAC40-111-210-A
Description: Based on record review, the minimal annual staff training was not obtained. Evidence: Staff #1 and #2 received 10 hours of annual training. 16 hours of annual staff training is required.

Plan of Correction: All staff will obtain the annual staff training this year. All staff will begin training this month.
