Comments:
An announced initial subsidy health and safety inspection (SHSI) was conducted on Thursday, March 23, 2023 from approximately 11:00 until 2:00pm. There were seven children present with three staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover all subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-580-A
Description: Based on review of five children?s records and interview, the vendor did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child could attend the center.
Evidence:
1. The records for Child #2 (first date of attendance 08/01/2022) contained documentation of immunizations dated 02/14/2023.
2. The record for Child #4 (first date of attendance 06/21/2022) contained documentation of immunizations dated 06/28/2022.
3. Staff #1 confirmed this information during the inspection.

Plan of Correction: Going forward, we will ensure all documents, including immunization records are received prior to allowing children to attend the center.

Standard #: 8VAC20-790-650-C-3
Description: Based upon temperature readings and observation, the vendor failed ensure that the running water at hand washing did not exceed 120 degrees Fahrenheit.

Evidence:
1. The temperature reading of the hot water at the hand washing sink located in the bathroom registered at 136.7 degrees Fahrenheit.
2. Seven children were observed using the hand washing sink after using the bathroom and before and after eating.

Plan of Correction: Water temperature on the water heater was lowered while the licensing inspector was still on site.
