Technical Assistance:
Reviewed again how to complete and document shelter in place drills. Discussed the importance of proper hand washing and supervision.

The following inspection took place between the hours of 9:15 am through 12:30. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 8 children (23 points) with 2 caregivers. A total of 3 children's files and 3 caregiver/household member's files were reviewed. The children were engaged in the following: breakfast, toy play, blocks, napping, diapering and snack. Standard #: 22VAC40-111-750-A
Description: Based on observation and provider interview, diaper ointment is not labeled with each child's name. Evidence: There were 3 different diaper crams found on the changing table, none of the containers were labeled with the child's name.

Plan of Correction: We do not use them, I will throw them out.

Standard #: 22VAC40-111-830-B
Description: Based on record review and provider interview, the shelter in place drills are not practiced twice per year. Evidence: There is no documentation that a shelter in place year has been practiced.

Plan of Correction: I will practice a drill and write it on the bottom of the fire drill form.

Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled with the child's full name or dated. Evidence: When the Li arrived there was one full bottle without a label or dated. The assistant then made a bottle for another child, the child drank the bottle, throwing it on the floor several times. The bottle was not labeled with the child's full name.

Plan of Correction: I will label and date the bottles.

Standard #: 22VAC40-111-120-B
Description: Based on record review, the provider has not maintained a complete record for each caregiver. Evidence: Caregiver #1, first day of hire, 3/13/17 does not have at least 2 references, no documentation to show that background checks have been obtained, and no documentation of TB within 30 days of hire.

Plan of Correction: I will get the required documents, I did send away for background checks.

Standard #: 22VAC40-111-50-A
Description: Based on record review, the provider has not maintained a written record of attendance each day. Evidence: There is no documentation that attendance was taken from 1/25/2017 through 3/6/2017.

Plan of Correction: I will take attendance each day.

Standard #: 22VAC40-111-180-A
Description: Based on record review, one household member has not obtained a TB screening every two years from the date of the last screening. Evidence: House hold #1 last TB screening is dated 11/25/2014, not within 2 years.

Plan of Correction: I will have him get a TB screening.

Standard #: 22VAC40-111-200-A
Description: Based on record review, the provider has not provided an orientation to the new assistant caregiver. Evidence: Caregiver #1 started 3/13/2017, there is no documentation that the caregiver has received the required orientation.

Plan of Correction: I will give her orientation.

Standard #: 22VAC40-111-560-A
Description: Based on observation and provider interview, the provider did not provide direct care and supervision while 2 children were in a separate room sleeping with the door closed. Evidence: Two children were observed being placed in cribs in a separate room,for over 45 minutes with the door closed. The children were not within sight supervision or checked on every 15 minutes, a baby monitor was not used either.

Plan of Correction: I will turn on the monitor each time a child is placed in the crib or check on them ever 15 minutes.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the caregivers did not use soap and running water to wash the children's hands prior to eating snack. Evidence: The LI observed the caregivers place the children at the table, cleaned their hands with hand gel and then gave them snack. The children's hands were not washed with soap and running water.

Plan of Correction: We will wash their hands with soap and running water.
