- Technical Assistance:
An unannounced monitoring inspection was conducted on 3/30/17 at 10:15 a. m. and concluded at 11:45 a. m. The inspector met with the provider and some but not all Family Day Home standards were reviewed. There was one child in care with points equaling two with the provider present. The child was observed having free play, and later the provider was working with the child on numbers. Two children records reviewed. Two household member records reviewed. The provider does not administer any medications. Conversations were held with one caregiver and one child. One observation conducted.
Standard #: 22VAC40-111-770 Description: Based on today's inspection the provider could not locate emergency equipment. Evidence: On 3/30/17, the provider could not find the weather band radio. Plan of Correction: I will continue to look for the weather band radio, and replace if needed.
Standard #: 22VAC40-111-60-B Description: Based on review of children records the family day home did not have all required information on file for some children. Evidence: On 3/30/17, child #1 record did not include all information required including Family day home hours of operation, holidays, check in and check out procedures, and the liability insurance disclosure form. Child #2 record was also missing grade and school child attends, and the second emergency contact phone number, also Family day home hours of operation, holidays, check in and check out procedures, and the Family day home provisions plan for emergency preparedness. Plan of Correction: Per provider, I will ask parents to get required information to complete children's records today.
Standard #: 22VAC40-111-170-A Description: Based on review of household member records the family day home did not have all required updates. Evidence: On 3/30/17, the tuberculosis screening for one household member was dated 3/20/15. Tuberculosis screenings should be updated every two years. Plan of Correction: Tuberculosis screening will be updated as soon as possible.
