- Technical Assistance:
1. Reviewed diapering and hand washing procedures. 2. Rocking chair and other seats should not be placed in the cribs for use.
Conducted an unannounced monitoring inspection 11:25am - 1:30pm. Observed 12 children = 37 points with the Provider, Assistant #1, and Assistant #2. Ratios and supervision were in compliance. Children were finishing lunch, having indoor free-play, listening to an audio book, and taking naps during the inspection. Caregivers were talking and playing with the children, and promptly tending to their needs. The daycare areas of the home were found to be very clean, safeguarded appropriately, and well supplied with toys and equipment for the children. New equipment noted. No medication reported to be on-site. Required documents were posted. Record keeping violations found. Questions about this inspection may be directed to: email@example.com
Standard #: 22VAC40-111-90-C Description: Based on records reviewed, it was determined that 2 of 11 children's records did not include immunizations updated every 6 months until the age of 2 years old. Evidence: 1. Child #3 - Date of birth 10/20/15, and immunizations on-file were dated April 2016. 2. Child #7 - Date of birth 11/24/14, and immunizations on-file were dated September 2015. Plan of Correction: The records have been requested from parents, and will be requested again.
Standard #: 22VAC40-111-680-A Description: Based on observations made, it was determined that caregivers did not wash hands after changing diapers and before touching other surfaces. Evidence: Caregivers opened the back door to dispose of dirty diapers before proceeding to wash their hands. Plan of Correction: We try to use wipes or hand sanitizer on our hands after the diaper change and before we wash hands with soap and water. Discussed with staff today to change the hand washing.
Standard #: 22VAC40-111-680-B Description: Based on observations made, it was determined that caregivers did not wash children's hands after changing their diapers today. Evidence: Child #8 was placed on the floor after having his diaper changed and did not have hands washed, and Child #15 was placed back in a crib after having her diaper changed and did not have hands washed. Plan of Correction: We try to use wipes or hand sanitizer on our hands after the diaper change and before we wash hands with soap and water. Discussed with staff today to change the hand washing.
Standard #: 22VAC40-191-60-C-2 Description: Based on records reviewed, it was determined that 1 of 2 assistant caregivers did not have a completed Central Registry Check (CPS) completed within the first 30-days of employment. Employment has not been denied. Evidence: Assistant #2 - Date of hire was 3/1/17, and there was no completed CPS check on-file. Plan of Correction: The CPS check was returned for incomplete information. We completed it and resent it last week.
