- Technical Assistance:
Consultation was given on removing pacifiers with stuffed animals attached (wubnubs) from the baby's crib once the child has fallen asleep as no stuffed animals should be in occupied cribs.
An unannounced two month monitoring visit was conducted during the Conditional licensure period. Four children were present with two caregivers and was within ratios for supervision. Children were transitioning from lunch, having free play and enjoying the nice day after being at the local park earlier in the morning. No medication was reported to be on-site. Lunch today was pasta with a beef sauce, strawberries, milk and water. Reviewed on e newly enrolled child's record, emergency drills, physical space and diapering procedures. Standard #: 22VAC40-111-50-A Description: Based on record review, the attendance log is not maintained each day. Evidence: A written record of attendance had not been completed for the past 6 days (4/3-4/10). Plan of Correction: Daily did it.
Standard #: 22VAC40-111-60-A Description: Based on record review, one of one records was incomplete. Evidence: Child #1's record was missing the hospitalization policy number and the documentation to complete verification of proof of identity (the provider did not sign and date the form). Plan of Correction: informed will be submit soon
