Fingerprinting will be required for all caregivers and household members soon - please look for e-mails with more information over the next few months. Code of Virginia 63.2-1720.1.D, effective July 1, 2014, states employees are prohibited from working in a position that involves direct contact with a person or child receiving services until a criminal record clearance has been received, unless that person works under the direct supervision of another employee for whom a background check has been completed. Direct supervision means that the employee will remain within sight and sound supervision of an employee with a completed background check.
An unannounced monitoring inspection was conducted with provider and two assistants. There were seven children present, ranging in age from 1 year to 4 years, totalling 18 points. The children were waking from rest, playing with age appropriate toys and preparing for snack. Snack served today: goldfish, fruit, and juice. Seven children's files were reviewed and are in compliance. Two staff records were reviewed - please see Supplemental Summary for missing items. The provider does not administer medication - Medication Administration Training is required to administer medication. Standard #: 22VAC40-111-50-A Description: Based on review, the provider failed to maintain a written record of all children in attendance. Evidence: Child A was not listed on today's attendance record (at approximately 3:00 p.m.). Plan of Correction: All children will be written on the attendance record within 30 minutes of arrival.
Standard #: 22VAC40-111-170-A Description: Based on review of two staff records, the provider failed to have documentation of a tuberculosis screening or test at time of hire of each caregiver. Evidence: Staff A (date of hire 3/29/2016) did not have documentation of a tuberculosis screening or test. Plan of Correction: Staff A will get a tuberculosis test or screening.
Standard #: 22VAC40-111-540-B Description: Based on observation, the provider did not exclude infant walkers from the day care. Evidence: there was an infant walker in the day care during today's inspection. Plan of Correction: Provider removed the infant walker away from the day care area.
Standard #: 22VAC40-111-690-E Description: Based on observation, the provider failed to have a foot operated, hands free diaper pail for diaper disposal. Evidence: the diaper disposal pail had a foot pedal, but it was not working during today's inspection. The lid did not lift when the foot pedal was pressed. Plan of Correction: Provider will get a new diaper disposal trashcan with an operating foot pedal.

Standard #: 22VAC40-191-60-C-2 Description: Based on review of two staff records, the provider failed to have an original search of the central registry(CPS report) within 30 days of employment. Evidence: Staff A (date of hire 3/29/2016) did not have documentation of a completed search of the central registry record (CPS check). Plan of Correction: Provider sent the CPS background check a few days ago.
Standard #: 63.2-1720-F Description: Based on observation, the provider failed to keep an employee without a completed criminal history record within sight and sound supervision of another staff person who has a completed criminal history record at all times. Evidence: Staff A was downstairs, alone with the children, when the licensing inspector arrived. Plan of Correction: Staff will not be left alone with children until the criminal history record check is returned.
