Standard #: 22VAC40-111-960-E
Description: Based on observation during the inspection, not all bottles were labeled with the child's full name and date. Evidence: There were two bottles that did not have names on them.

Plan of Correction: I will put the labels on when the parents forget the name band.

Standard #: 22VAC40-111-60-B
Description: During a review of a sample of children's records today, one file was missing some information. Evidence: Child #1, in care over seven days, did not have proof of identity verified and had an incomplete parent agreement related to Provision of Emergency Preparedness.

Plan of Correction: We will have her mother complete the paperwork.

Standard #: 22VAC40-111-240-A
Description: Based on observation today, an area of the home was not maintained in safe condition. Evidence: In the kitchen today, kitchen knives were left on a table after use when children were present and the gate was unlatched between the kitchen and living room.

Plan of Correction: They had been used to cut grapes. They were removed.
