Technical Assistance:
Discussed not using the patio with the children, discussed requirements for new employees, and updating emergency plan.

An unannounced monitoring inspection was conducted today from 12:00pm-1:15pm because the provider relocated to a new home. There were 7 children (18 points) directly supervised by 2 caregivers. The home, 2 staff records, 2 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and preparing for lunch. Handwashing procedures were also observed. There was a supply of books, toys, and materials for the children. Standard #: 22VAC40-111-800-A
Description: Based on record review, the provider did not have a completed written emergency preparedness and response plan. Evidence: The provider did not update information in the emergency preparedness and response plan to outline the primary and secondary routes for the evacuation and shelter in place drill for the new home.

Plan of Correction: I will update the emergency plan.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not ensure that all areas and furnishings, inside and outside, were maintained in a clean and safe condition. Evidence: In the backyard, the bottom of the fence has broken boards exposing sharp pieces of wood. There are also fairly large holes which can pose a danger to children. In the backyard, two bicycles lost the rubber handle bars exposing two holes in which children's fingers could get caught.

Plan of Correction: I will put wood to cover the wholes. I will buy handles for the bike or cover the holes.

Description: Based on observation, the provider did not unplug all small electrical appliances while not in use. Evidence: A toaster was plugged in while not in use during the inspection.

Plan of Correction: The toaster was unplugged at the time of inspection

Standard #: 22VAC40-111-430-A
Description: Based on observation, children had access to stairs. Evidence: The stairs leading up to the patio from the backyard did not have a gate in place to prevent access to children.

Plan of Correction: I will put a gate at the bottom of the stairs.
