Technical Assistance:

An unannounced monitoring inspection was made on June 6, 2017 at 11:30 a. m. and concluded at 1:00 p. m. The inspector met with the provider and Family Day Home standards were reviewed. There were eight family day home children present with points equaling 15, with two caregivers. The children were observed having free play on the fenced in playground. After outside play, the children came into the home and one caregiver had circle time and the children sang songs, month of the year, and dancing to music. The children were served corn dogs, tater tots, cole slaw, cucumbers, and milk. Three children records were reviewed. Conversations were held with the provider and several children. Observations were made in the preschool room and outside. No children on medications today. No new caregivers, all records were up to date. The last monthly evacuation drill was conducted on 6/2/17. The last shelter-in-place drill was conducted on 4/3/17.

Description: Based on review of children records the family day home did not have all required information on file. Evidence: On 6/6/17, child #1 and #2 records did not have the policy number for insurance. Child #2 record did not have a phone number for one emergency contact and there was no second emergency contact, address or phone number available.

Plan of Correction: I will get the information as soon as possible for child #1 and #2.
