06/01/2017 12:20am Departed 2:30pm An unannounced monitoring inspection was conducted today. Eight children were in care for a total of 14 points. Children's and staff records were reviewed. Standard #: 22VAC40-111-460-C
Description: Based on the review of records and an interview with the provider, the facility failed to repeat rabies vaccines for household cats. Evidence: 1. The vaccine records for cats #1, 2, 3 expired on 5/4/17.

Plan of Correction: The cats will be vaccinated next week.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on the review of two staff files, the facility failed to renew a criminal background check. Evidence: There is no repeat criminal background check on file for staff #2.

Plan of Correction: Staff #2 will complete a background check tonight.
