Comments:
Today an unnnounced 2 month visit was conducted with the Provider. This visit took place in the morning, there were 5 children present with the Provider for a total of 12 points within the supervision guidelines. The children were observed having morning free play, puzzles, blocks, morning snack, nap and lunch time. Lunch consisted of chicken and rice, cucumbers, strawberries and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, previous violation cited, children paperwork, attendance, and menu were observed during this inspection. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of the children records, the following information was missing: parents work phone number, medical insurance policy number. Evidence: Child #5's record did not include the child's medical insurance policy number.

Plan of Correction: Provider stated that she will call the parents and ask for the information.

Standard #: 22VAC40-111-320-B
Description: Based on observation of the physical plant, electrical outlet was found missing protective cover. Evidence: Upon arrival, an electrical outlet near the door was found missing a protective cover.

Plan of Correction: Provider covered it during this inspection.

Standard #: 22VAC40-111-560-A
Description: Based on observation of the children, a caregiver was not observed physically present while an infant was sleeping. Evidence: Upon arrival, 4 children were observed outside on the porch playing with a caregiver. Provider came outside within a minute, however an infant was sleeping in a crib and the provider did not have a baby monitor outside to monitor being able to hear and see them.

Plan of Correction: Provider stated that she will purchase a baby monitor and stated that she checks on the baby often.

Standard #: 22VAC40-111-680-A
Description: Based on observation, childrens hands were not washed with soap and water before feeding them. Evidence: Children were observed being served fruit snacks however theirs hands were not washed first.

Plan of Correction: Provider took children inside to wash hands and then gave them back the snack. Provider stated that she usually wash hands before lunch.

Description: Based on observation and interview with Provider, the most recent inspection was not posted. Evidence: Upon arrival, the most recent violation notice dated (4/11/17) was not posted in the childcare area.

Plan of Correction: Provider posted it during this inspection.
